Early implementation of Fundamentals of Endoscopic Surgery training using a simulation-based mastery learning curriculum

Background After the American Board of Surgery announcement Flexible Endoscopy Curriculum requirement in 2014, we implemented a dedicated endoscopy rotation at post graduate year (PGY)2 level including simulation curriculum for Fundamentals Endoscopic skills. Here evaluate outcomes this implementation. Methods Beginning 2015, developed clinical and simulation-based to prepare testing. Originally, our was based on published Texas Association Surgical Skills Laboratories using GI Mentor transitioned mastery learning Training System 2016. We evaluated success terms first-time pass rates, training time required, comparison previously benchmarks experience. Results Since total 37 general surgery residents program were tested (PGY2 = 24, PGY3 4, PGY5 9); 84% (31) completed curriculum. At testing, 73% (27) had performed <25 esophagogastroduodenoscopies, 46% colonoscopies. Ninety-two percent (34) spent 10 hours or less completing The rate those 97% vs 67% not (P .01). For curriculum, scores discernibly higher (472 389, P < .01), as 3/5 task (Nav1 80 67, .02; Loop2 36 8, Retro3 89 71, .02). Despite inexperience (<25 esophagogastroduodenoscopies <50 colonoscopies), PGY2s yielded mean score 454 92%. This similar PGY5s (427, 89%; .3) compares benchmark data endoscopists with >300 cases. Conclusion Early implementation flexible results performance equal experience often gained during surgical residency. Often requiring <10 hours, represents fantastic return investment training.
